Could still be airways: Chronic sinusitis need to be ruled out together with other causes for cough, as asthma , chronic middle ear infection or reflux. On occasions you can have pneumonia without fever, may be, but you need to have rapid breathing/shortness of breath, not just cough, chest x-ray would confirm diagnosis, see his doctor again for re-evaluation
