Tough one: Because you are on Immunosuppressants, gp was concerned about pneumonia, which is usually accompanied by cough, phlegm, and fever. Can't tell from what u say what u have. So many causes of chest and shoulder pain. Most obvious thing for you would be something related to RA. If it hurts when u move your shoulder this would be yet more likely. Best wishes.
