Associated signs?: Most important would be if there are signs of redness, heat, asymmetry, skin rash associated with the periareolar pain--all these might suggest infection. If none of these, most likely explanation is cystic or fibroglandular change. The more reassuring news is that cancer is often NOT associated with any pain. However, after a month, best course is to have your doctor evaluate. Good wishes:)
