The "hole": below the belly button is likely a dimple that is the result of the uterus pulling higher up to accommodate the newly formed placenta. That should not be anything to worry about. Back pain and swelling of lower extremities are expected to occur in the second trimester, along with other things like weight gain, difficulty breathing (though usually worse in 3rd trimester), and constipation.
