Good question!: First, if there is a condition that has to be fixed. Viral infections are by far the most common causes of nausea/vomiting/diarrhea but if you're dehydrated/can't keep anything down, doesn't matter, needs ER. Likewise respiratory or other symptom that needs fixing. Severity, duration, intrusiveness of symptoms. Gunk is mostly bacterial, mostly needs addressing.
