Mid back pain: Without more clinical information, it's difficult to say what the precise diagnosis is. Based on the limited information, it does sound like he is experiencing some sort of irritation or inflammation of his spine. It could be as benign as muscular strain and as serious as a thoracic cord compression. If he's experiencing symptoms down his legs, i would have him seek a formal medical evaluation.

Spinal radiculopathy: It sounds like a pinch spinal nerve from either a slipped disc or osteophyte off the spine itself pinching the nerve root he should check that out.
