Distended GB: is normal if he is not eating anything by mouth. The emesis could be from multiple things, since I am assuming there are other health issues given he is on TPN daily? However, GB trouble can cause emesis, although usually the main symptoms are right upper quadrant abd pain after meals, nausea, bloating, indigestion. He needs further workup. Good luck!

Mgt: There are many conditions which may cause gallstones. Among these are Hemolysis, Cystic Fibrosis and high cholesterol/obesity. Please check with your PCP who can advise you regarding testing and treating underlying causes.
