Depends: My criteria to begin to determining the severity of the injury is what happened and if there was loss of conciseness. Bike injuries vary in severity depending on the speed at which the fall happened and how the head was injured. If he was not knocked out and appears to be normal, watch him closely for 4 hours. May be okay: no problem if no bleeding, no wound, cried immediately, did not pass out, did not had nausea and vomiting and now is acting normally,
