Not necessarily: Doses as high as 120 mg of mirtazapine have been reported in the literature. More important, is the 90 mg dose prescribed or is this amount being taken to attempt self-harm? Not uncommon for some prescribers to push towards higher doses, while others stay lower. All may be reasonable if the patient benefits. A prescribed dose should be well-explained and justified. Call poison control if necessary
