This is hard to explain so i will try my best. my nipple seems to have a 'hole' at the end of it in the centre and it looks raw and sore. no visible discharge but inside the hole looks red and dotty, like tiny little blood spots have come to the surface o

Below: Difficult to give DX from the history, but I would suggest trying some warm, moist packs to the area 3-4 times per day, about 15 min each time.
If further concerns or new symptoms, then exam is needed
