Many possibilities,: impossible to tell with any reliability from a distance, likely difficult even close up & bad info often harmful. Anatomy at location, from surface in: skin, subcutaneous fat, abdominal wall muscles (several layers), omentum, small intestines, etc. However, given embryologic development & nervous system design, pain sensation could easily be referred from another location, other organs/anatomy.
