Probably Normal: Without viewing your EKG and knowing more about the characteristics of the R-wave (QRS duration, etc) it is difficult to say for certain but most likely represents a nonspecific intraventricular conduction delay. In an otherwise healthy asymptomatic 20 year old without structural heart disease it is an interesting incidental finding of no clinical significance.
