Doubt chlamydia: Did you have a positive lab test for chlamydia before treatment? Chlamydia is rare in men your age (45), azithromycin is always effective, and chlamydia rarely causes burning without dischrage. Sounds like a different problem, perhaps a prostate problem, pelvic pain syndrome, or non-STD urinary tract infection. Azithro doesn't work for these. See a urologist or infectious disease specialist.
