Some do: About 20% of women experience mittelschmerz which is a mild pain at the time of ovulation. Some patients may have a small amount spotting at that time. It is thought to be related to swelling of the ovarian follicle and due to the release of a chemical called Prostaglandins. Taking birth control prevents the pain.
