What can be the reasons for unbearable scrotum tightness and shrivling? can constipation have any connection to this?

Not common: I've been caring for patients for over 35 years and I don't think I've heard this one yet. An exam is in order. I would wonder about torsion, epididymitis, Varicocele inflammation, an inguinal hernia but I would not associate it with the constipation. Straining however might aggravate a hernia.
