See a surgeon: What exactly have you tried? for how long? If you have given conservative treatment options a good trial to see if your anal fissure will heal and it is not getting beter, then it is time to see your surgeon to discuss surgical options. There are surgical options to help you if you have reached the point that you can't heal the fissure nonsurgically and are willing to have surgery for this.
