Palpitations: These are caused by many things. Stress, excitement, irritants both physical and chemical, pressure like a hiatal hernia or esophageal push or pull, and primary heart things, the causes are very many in number. Esophageal spasm feels like a grab in the chest and lasts as a single feeling longer than palpitations each one of which is very s short.

How do you tell the difference between an esophagus spasm and a heart palpitation?

See below: Esophagus spasm would be more likely to produce pain whereas palpitation is usually a fluttering sensation. The word palpitation refers to the feeling in the chest but it usually implies that the problem is an irregular pulse. An EKG would determine if the heart rhythm is regular or not. Sometimes we have to hook the patient up to a 24 hour monitor to catch the irregularity.
