No: While a Spitz nevus is seen mainly in younger individuals it is totally benign. It needs local removal but not the wide resection that is normally performed with an aggressive melanoma. If one is completely sure that this is a Spitz lesion then local resection of the primary nevusis all that is required.

Sentinel Node bx: Spitz nevi are not common in adults, and occasionally have some overlap features with melanoma. For this reason, sentinel node biopsy has been performed to determine if any early evidence of malignant behavior (LN metastasis) has occurred.
