Physical exam: It sounds like the doctor was describing their findings on the physical exam. The introitus is the opening of the vaginal. If the term virginal introitus was used I suspect they were describing an intact hymen -- meaning the physical exam was consistent with a women who had not yet been sexually active or had disruption of the hymen.
