Microscopic look: Grade refers to how cells look under the microscope. If they look like the natural cells in the organ they started from except more crowded they are grade 1 or well differentiated. If they look like solid sheets of nasty cells with no appearance of normal tissue, then it is a grade 3 or poorly differentiated. A combination of the 2 is called grade 2. Usually, the higher the grade the worse it is.
