How accurate percentage-wise can a doctor be in pinpointing osteochondroma as opposed to malignancy by looking at initial x-rays? no ct or mri yet

Usually accurate.: Plain radiograph appearance of osteochondroma is typical and usually quite accurate for determining benign process. Malignant degeneration of osteochondroma is rare but usually involves sudden growth and change in appearance of lesion and associated pain. Serial radiographs without interval change speak of benignancy.
