does a patient always have to have congestive heart failure as a diagnosis when given a nitroglycerine trans dermal patch for stable angina?

No: NTG patches are used to prophylax angina and CHF is not a prerequisite.

YES: But close monitoring by the cardiologist is a must. And any other meds RXed by other DR's need to be also have cardiology approval.
