Often not known: Some have a high threshold for pain, others don't. Ur correct, some lucky women experience no pain with periods. Very severe menstrual pain can be associated with endometriosis or pelvic inflammatory disease. Pain is from uterine contractions which which vary in tensity from woman to woman. Possibly severe contractions in some, briefly reduce blood supply to uterine muscle causing increased pain.
