Ear infection/pain : Usually infection in your middle ear can cause sever pain, if you have outer ear infection you will have pain as well. The only time you cna have middle ear infection and no pain is when the pressure build up in your middle ear causes the ear drum to rupture & the pressure is released and you will have no pain.

Yes: The infection of a middle ear is caused when germs eat the mucous of the middle ear.Although it may eventually cause a stretch of the ear drum, which is the only thing that causes the pain, if early or very slow moving, they can infect for a considerable period without causing pain.
