A "shadow": It refers to something which shouldn't ordinarily be there! A number of conditions could cause this finding which is "NON-SPECIFIC"...that is it needs more studies (for example a CT scan) to "name it"...By the way "vertical means "up and down"...The most common vertical shadow behind the heart (Retrocardiac) is the THYMUS gland ...which has enlarged...Hope this helps!
