Efficacy: Antibiotics are not likely to be necessary or effective in treating an infection which occurred five yrs ago. No testing has shown persistence of the the organism for that time period, although other spiral bacilli may do that (syphilis). The basic question is whether the symptoms your daughter has are truly related to the prior lyme infection? See a doctor with experience in this illness.
