Herpes unlikely: Herpes can cause lesions similar to anal fissure, but the large majority of anal fissures are not herpes. However, if the fissure is new (within a few days), it is difficult or impossible to tell for sure only by symptoms. Initial herpes usually causes multiple lesions and often fever, etc; but maybe no other symptoms with recurrent herpes. See your doctor for diagnosis and treatment.
