Sometimes: While humans lack the enzyme necessary to split the double sugar moleculae, lactulose, into two single sugars, sometimes, their gut bacteria have this enzyme, so that some people do see their blood sugar go up with lactulose. People who need lactulose for ammonia build-up due to liver disease don't really have an alternative. There are other options for constipation if it makes your sugar go up.
