See a urologist: your freind need to see a specialist his condition is associated with high risk of testicular cancer,if he had undescended testicles. if his testicles were underdeveloped because of undescended testicles the he need a full evaluation of the level of his testosterone level to determined if supplement will be sufficient or it is too late. this is a serious and complicated problem. see a specialist.
