New back pain: Unfortunately, new back pain that is not improved after 2 weeks and worse with sitting is suspicious for a herniated disk in the lumbar region. A physical examination will probably confirm this and help to grade the severity. Imaging may or may not be needed. 90% of patients improve within 6 weeks, so conservative treatment is probably still best; but a physical exam is needed to know more.
