NeedTheBloodyDetails: Is this person a vampire? If, not, then I can't imagine anything acceptable about blood coming out of the mouth, especially when talking. Now you said "microscopic," which means to me that you can't see it and need a microscope to see it. So, what's with you? How did you see it? Carry a microscope around with you? Anyway, my guess is the gums but it could be lots of things so see a dentist first.
