Accutane dosing: A reduced initial dose of Accutane may reduce the flare that sometimes occurs with Accutane. However, you need a certain total dose of the drug per course and by starting at a lower dose, you are lengthening the course of treatment in order to get the full dose of the medication

Wrong question: I think you are assuming that acne gets worse before it gets better. This is not the way it works. Low initial dose may help long-term tolerance. Full dose, with worse dry lips and skin, is more likely to result in permanent cure. Small dose still is very effective to clear acne.
