ENT Evaluation And: I will suggest you consult a ENT Specialist to get your Sinus issues treated appropriately since you have severe facial pains It may be from the sinusitis or there is possibility of Trgeminal Nuralgia depending on what part of face is painful. Can only be diagnosed after taking detailed History and Physical Exam and if indicated imaging and other teasts

TMJ: Sounds like TMJ. See ENT doc, if negative for ENT issues, see TMJ doc. Any dentist can be a TMJ expert with the proper training and experience. Most commonly, oral surgeons, prosthodontists, and orofacial pain specialists. Ask your MD, your dentist and your dental society for referrals.
