Tenesmus: He should be evaluated by his pediatrician - along with a physical exam, his physician knows more about his medical history and is the best person to provide a diagnosis. It could be that he's just constipated and having a difficult time passing stool, but it could be something more serious and only a hands-on physical examination is going to be able to provide the answers.
