Sinuses: Pain behind your eye intermittently for 3 years is most likely sinus related. Please see your health care provider to get a confirmatory diagnosis and treatment for this.

R/O CLUSTER HA: Cluster Headache are more common in men and tend to be felt behind the eye. They can be treated with Triptans like imitrex/relpaxand high flow Oxygen for flare. If you get more than 3-4 month. You will need a prophylactic med like neurontin/depakote/Topamax . You need a full exam and Neuro exam. Labs and an Mri of brain and sinuses. To rule out other causes Of unilateral headaches.
