Blocked skin pores: either from friction or rubbing on forehead or from internal factors which are causing a change in your body's natural oil or sebum such as diet in saturated fats or change in hormones Taking any supplements? Suggest starting with benzoyl peroxide OTC. See your doctor if not helping for further evaluation.

Adult acne: Adult acne can occur due to certain medications such as hormone replacement therapy/ birth control pills and steroids. Also, hormone fluctuations that occur around the time of menopause can cause outbreaks as well. Forehead acne may be a result of blocked pores from a hair product. Look for hypoallergenic oil free pH balanced facial cleansing products.

How severe?: If it is significant you will get best result from evaluation by a dermatologist with appropriate prescription. Since you are a 32 year-old man, it is not related to menopause or any meds used for menopause.
