Need more treatment: A positive test means that you still have an infection. This may mean the the antibiotic course was not long enough or that the organism developed resistance to the azithromycin. Talk to your doctor about other options. You need to be treated ASAP.

It depends: Was your partner treated as well? Are you using condoms for all sexual intercourse? If you took the medication properly, it will probably clear up the infection, but if you get reexposed by continue contact with another person who has chlamydia, you will get it again.

Very unlikely: Azithromycin is highly effective against chlamydia; there is under a 5% chance (1 in 20) this test will be positive. However, your doctor or clinic may recommend you have yet another test at 3-6 months. Late treatment failures are more common than early ones, and many people become reinfected. Therefore, late retesting is recommended routinely in the US; I don't know UK policy on this.
