Lots of reasons: Depending on the specific STD, the first person tested might have a false positive, or the second a false negative result. Maybe partner not infected yet, if other partners involved. Most STDs are cured over time by the immune system, so the partner might have self cured. Or took an antibiotic, either for the STD or something else. Both partners should be treated, including the one with neg test.
