Urologist: First the left testicle usually will hang lower than the right. Why this month I am not sure but may be you have not noticed it until now. Needs an exam to see why you have pain. Have you had trauma and other issues could involve infection such as orchitis. See your doctor.

Infection or torsion: It may be an infection or testicular torsion. I recommend going to the emergency room now.
