A very specialized: hospital taht would take referrals from other hospitals. Generally they are ones in large cities, that have medical schools attached to them and do research. Often called "teaching hospitals". Also, specialized diagnostic and therapeutic functions need to draw from a very large area to support themselves (if you only get one heart transplant a year, you cannot keep the facilities available.
