Fever or > a week: Most sore throats are caused by viruses, even if they are severe, and will go away on their own. If a sore throat is accompanied by fever or swollen glands, it may suggest a strep throat, which is bacterial and needs antibiotics. If a sore throat last more than a few days and no other symptoms like stuffy nose or cough develop, it could be strep. Doctors can test for strep and treat it.

3-4 days: A sore throat without fever or other symptoms in an otherwise healthy young person should be self limiting in 3 or 4 days. Drainage can be irritating your throat. If this persists you should see your healthcare provider for further assessment of other causes.
