Acne vs Impetigo: Hard to say without a photo for diagnostic purposes. Women do get peri oral dermatitis, an acne which affects the mouth area. But your description of the crusting sounds a lot like impetigo, commonly caused by staph. We could make a better determination with photos and deeper history during a prime visit! Treatment is different 4 each condition!

See dermatologist: Probable acne, but exam may reveal other diagnosis such as perioral dermatitis, folliculitis, impetigo, contact dermatitis, etc. Most likely success with oral spironolactone.
