Paradoxical embolus: is an uncommon cause of embolic stroke. Straining on the toilet can create the pressure gradient necessary for the clot to cross from the right to the left side of the heart through a patent foramen ovale. The clot may take days to weeks to build up in the leg vein prior to this.
