Multiple causes: The most likely cause is spasms of the pelvic floor muscles. During orgasm this muscles will have rithmic pleasurable contractions. In patients with vulvodynia, ic or endometriosis these muscle could become hyperexcitable and makes them cramp like a charlyhorse. This pain may last for minutes, hrs or days.
