You've answered the?: If the cortisone shot gave so little relief in time, the tear will not heel itself. You'll be miserable sleeping. It will impair activities of daily living. Talk with an orthopedist! sooner than later!

Depends: If the pain and disability are not tolerable it may be time. You need to discuss this with your orthopedist.
