High bp in office: Blood pressure is commonly elevated when you get it checked in your doctor's office -- it's a well documented phenomenon. Home bp's are much more accurate. It has nothing to do with your medical condition or rx. It's seen in many people.

White coat HTN: White coat hypertension is a situation whereby blood pressure is normal at home but elevated at the doctor's office. This can be confirmed by recording your measurements at home, then bringing in your machine for calibration in the office (measure your BP side by side one after other in sequence w/office BP cuff). That will allow for interpretation of home readings.

Home BP DIFF OFFICE: Most likely "white coat" HTN. But how is office taking ur BP? Exactly the same as u do at home? Have them watch u use ur device in the office to see if numbers same as home. If higher in office with ur device then WC. Does your device read lower than what they hear when ur device takes pressure. Both using same size cuff? Same brand device used in off as u use? Standard rest 5 min take 3 average
