Double check: Make sure the infection is cleared with another urine analysis and culture. This is the most common cause of persistent symptoms. Pyridium (phenazopyridine) helps with acute symptoms but it is not for chronic use. It is possible with a severe infection that there is significant bladder inflammation that takes a while to resolve but i would bet persistent infection with a bacteria resistant to original antibiotic.
