Family wishes: If the death is not a public health concern or one that must be investigated because of suspicion of homicide or because there will be medicolegal implications, in most jurisdictions the family can decide. As a pathologist i encourage families to have the procedure done and am sorry that it's more difficult to get nowadays. An embalmed body can be autopsied years later.

Cost & infection: A family can request an autopsy if the person died in a hospital. However, the rate of autopsies has decreased, in part, because hospitals do not get paid to do autopsies. An autopsy can be done before a body is cremated. Some times hospitals may not do an autopsy if there are no questions about the disease and/or if the person has a communicable disease.
