Pain: Sometimes people continue to have sciatica pain because the screws have loosened or an infection has developed. Sometimes it just takes a long time for the nerve pain to diminish (particularly if the pain was present long before surgery. Either way be sure to follow up with your surgeon to evaluate your back to ensure everything is healing appropriately.

Many reasons : If you had sciatica prior to surgery it may simply be residual. If it was not there prior it could be due scar tissue or inflammation especially if the surgery was recent. Additionally new disk problems can occur next to spinal fusions. Lastly, if hardware (screws and rods) were used it is always possible thus could irritating your nerves. Difficult to answer without more details.

Residual lat. recess: Spine surgeon follow up. May need MRI to look at the lareral recesses of the spinal canal where a nerve may still be pinched.

Wear/Tear: With previous back surgery, sometimes pain can result from the wear and tear of the levels above/below your surgery that takes place after a fusion surgery. If this is the case, recommend seeing your spine surgeon or a spine/pain specialist to evaluate this further.
