May be tendinitis : If you are describing pain that starts behind the thumb and radiates up your forearm when you make a fist, then you probably have a dequervain's tenosynovitis. This condition is notorious for not responding to many treatments besides a cortisone shot into the first extensor compartment or surgical release if the injection doesn't work.
